What evidence is there that the design of Step 2 CS is sound? How many people pass Step 2 CS? Will I be able to learn how I performed on specific areas of the exam? We are not yet able to provide this feedback without compromising exam security, but we are working toward that goal. We know that test-takers would like the opportunity to learn more about their performance on this important component of the exam. Why are there so few testing sites for Step 2 CS? While additional sites will continue to be considered to further lessen any inconvenience and financial burden, a significant limitation has been the intensive cost to the program and ultimately examinees of maintaining additional secure facilities.
An additional site in Philadelphia was added to address demand from the busy mid-Atlantic region and the program will continue to monitor demand and current capacity to meet the needs of examinees. Why doesn't the USMLE program use some of the excellent clinical skills facilities at universities to expand testing sites? The test sites used for administering Step 2 CS are consistent in their physical layout and their operational components, such as security features, staffing, training for standardized patients, scheduling, and so forth.
This consistency is essential to ensure all examinees have an equivalent experience regardless of where they test. The upfront and ongoing expenses to the USMLE program and, ultimately, to examinees of adapting medical school sites to provide a uniform testing experience are not practical. How far in advance do I need to register and schedule to find available appointments at the test center of my choice?
When you apply to take Step 2 CS, you are assigned a month eligibility period that begins when processing of the application is completed. Schedules at all test centers fill up quickly. We strongly encourage examinees to complete their scheduling before May 31 of the calendar year in which they plan to test. Testing appointments are typically completely filled at least three to four months in advance. If you try to schedule after August 1, you may find that there are no available testing appointments through the end of the year unless there are cancellations. How are the patient notes scored?
Patient notes are scored using a hybrid system of trained physician raters and computer-assisted scoring. Patient note raters are licensed, board-certified physicians, and have experience in medical education. Occasionally, due to technical or administration problems, typing the patient note may not be possible for one or more patient encounters. When this happens, examinees will be required to write their patient notes by hand. This problem is rare, but it can happen. All examinees should be prepared for the possibility that they may have to write one or more patient notes by hand. Handwritten patient notes are rated by physicians who are thoroughly trained at reading notes and can interpret most handwriting. However, extreme illegibility will be a problem and can adversely impact a score.

The width of the bands is based upon measurement precision, and is intended to reflect the approximate variation in scores, both higher and lower, that would be expected to occur if an examinee were tested repeatedly using comparable sets of clinical skills cases.
If a subcomponent is failed, but the band of X's extends into or beyond the "borderline performance" area, this does not reflect a mistake in scoring but indicates a relatively high failing performance. The CIS checklist used by the standardized patients allows them to assess examinees in crucial communication skills areas, such as fostering the relationship, gathering information, providing information, helping the patient make decisions, and supporting emotions.
